Mortgage Checklist

This following checklist of items was created with the main goal in mind of maximizing the efficiency of the approval process, while at the same time minimizing the time it takes. 

The following items are needed for all conventional purchase and refinance transactions.

  • W2 forms for the last 2 (two) most recent tax filing years.
  • Current computer generated pay check stubs for the last 2 (two) consecutive pay periods. Should include current rate of pay ie: hourly, salary, commission and year to date earnings.
  • Actual statements for liquid checking and savings accounts from bank or credit union (please include all pages) for the previous 2 (two) month periods.
  • Investment account and IRA (401K) statements (include all pages) for the past two month periods.
  • Bring Wisconsin driver's license to application appointment for copying.
  • A complete copy of the signed and accepted offer to purchase on your new home.
  • Names and contact information of listing and selling real estate agents involved in the transaction.
  • If divorced, paying alimony or child support, or receiving alimony or child support please bring in for copying a complete divorce decree along with the marital settlement agreement or paternity agreement.
  • If refinancing please bring in your current mortgage statement from the last month paid.
  • Proof of home owners insurance including dates of coverage and premium amount.

 If you are self employed or receive more than 25% of your earnings from commissions please provide the following documentation.

  • Sole proprietorship: complete personal federal 1040 from previous 2 (two) most recent filing years.
  • C or S Corp: add 2 (two) most recent years federal corporate returns (along with personal returns)
  • Partnership: add 2 (two) most recent years federal partnership returns and K1 schedules as applicable (along with personal returns)
  • Provide CPA or bookkeeper name and contact information.

For FHA and VA loan applications.

  • Bring in social security card for copying.
  • VA Certificate of Eligibility.
  • VA discharcge form DD214.
  • If any portion of the down payment funds are coming from a family member in the form of a gift, please provide the name, address and phone number of the gift donor.
